Position Summary:

Provides individualized, goal-directed care; delegated by and under the supervision of a Registered Nurse or Licensed Practitioner through the application of the nursing process, in accordance with departmental and hospital policies and procedures.   Under the direction of the RN the LPN is responsible for patient assessment, documentation of assessment findings, interventions, and outcomes.  The LPN will collaborate with the interdisciplinary team on patient discharge plans and patient/family education to provide continuity of care.

Education requirements:

Completion of high school diploma or by equivalent outside study.
Have a current license to practice as a Licensed Practical Nurse
BLS certification

Ability to apply knowledge of patient rights, safety rights, safety practices, infection control principles, and risk management issues.  Ability to follow policies and procedures of department and hospital.  Ability to maintain a high level of integrity, confidentiality and professionalism.  Serve on committees and assist with programs of the hospital to constantly improve the quality of health care and promote good public relations.  Ability to work in a timely manner, within the time allotted for the shift per patient census.
