Our swing bed services provide short-term, comprehensive care in a hospital setting, giving you easy access to skilled nursing and therapy care. As a swing bed patient, you will be encouraged to do as much as you can for yourself.
Experienced Team
Our experienced team of healthcare providers will work closely with you and your family to establish an individualized recovery plan that meets your needs and assists you in regaining your mobility and independence.
Healthcare Providers coordinate your care and make weekly visits.
Nurses assess and closely monitor your progress, dispense medication and provide treatment.
Physical, Occupational, and Speech Therapists help you regain your optimal level of functioning.
Dietician consultation on every admission.
Social Worker assists with emotional support and discharge planning.
Pharmacy staff reviews medications and provides education.
Respiratory Care administers treatments and pulmonary care.
Admission Requirements
For Medicare coverage, you must have a three-night acute care hospital stay within the last 30 days.
Must have a skilled need (occupational therapy, physical therapy or skilled nursing).
Willingness to participate in an active medical and physical program based on your needs.
